We have a one-time, non-refundable registration fee of $75 per child or $150 per family. (Cash, Money Order, Or Credit Card via Tuition Express)
Enrollment fees may be mailed to 8755 Upper 208th St W, Lakeville, MN or completed via our Tuition Express Payment Authorization Form.
Download and complete the Enrollment Package Here. Please complete this package in its entirety as it is required to be on file at the center by your child’s first day.  When filling your enrollment forms please remember a minimum of 2 emergency contacts and 2 authorized transports must be listed.
Please also print and complete the Health Care Summary, Student Information Sheet, and Immunization record.
Hypointe Childcare and the Minnesota Department of Human Services Licensing Division require that all children have a current Health Care Summary and Immunization Record on file prior to the start of care. These forms must be filled out by your primary care physician and can be faxed to the number listed on the form. You will not need an appointment with your Pediatrician for this, most take 3-5 business days to send information requested.
